Full time case manager position available, Duties include but are not limited to:
a. Adheres to established regulations, policies, and procedures of the overall agency and their specific program.
b. Attends trainings to enhance skills and job performance as directed by program supervisor.
c. Performs in an advisory capacity to other staff in specialized areas through participation in meetings for the purpose of treatment planning.
d. Provides outreach to community organizations and referral sources regarding agency programs as directed by the Program Supervisor.
2) CASE MANAGEMENT/DIRECT SERVICE
a. Provides weekly/bi-weekly case management/supportive services to residents while at the shelter, including developing and implementing an individual service plan with all residents.
b. Provides supervision of residents while at the shelter, as well as during any outside events, appointments, etc.
c. Provide intake and admission duties for residents coming into the shelter. This includes screening individuals for program appropriateness (Megan’s Law List etc.) and gathering demographic and other information.
d. Work in close cooperation with referral sources and other social service agencies in planning to meet the needs of clients and assists the clients in utilizing their resources.
e. Prepared and maintains records as required by specific program and state regulations, on a timely basis, including entering data in the HMIS.
f. Performs related duties as requested and assigned by the Program Supervisor or Director including drop in center coverage if necessary and assigned.
3) SUPERVISION
a. This position will report to the Program Supervisor for direction, monitoring, and evaluation of job performance.
QUALIFICATIONS:
This position must possess a Bachelor’s degree in a Human Services Discipline, possess a valid PA driver’s license and must be able to navigate stairs and be able to lift at least 50lbs.
